Update your records

In order for the practice to be able to contact you quickly and easily, patients are required to keep their records up to date.

If you move address, change your mobile number or email address please contact the practice via PATCHS to let us know.

If records are not updated, this may delay referrals and delay the practice from providing you with the medical reviews that your health or medical condition may require.

Any change of name must be accompanied by the original document which confirms the name change.

Please note, the practice will contact you for de-registration if you new address is outside our Catchment Area
