New patients prescription request

New Patients Prescription Request

All new patients undergo a new patients’ health check.  It will be advisable to obtain a summary print out of your medical records from your previous GP when attending for the health check.

Unfortunately, bringing empty boxes of old medication will NOT be accepted if requesting for a repeat prescription.  Please NOTE, a GP requires information on your medical condition and to see what previous medications you have been on. The ONLY way to obtain this information if a patient’s records have not yet been received by this practice is from the Brief Summary Medical Report mentioned above.
