Registering with The Practice

When the Registration of the patient is complete, the new patient will be invited for a New Patient check.

Advice on completing the registration form

Patients failing to FULLY complete the GMS1 form for Patient registration will find it rejected.  Patients are requested to read the New Patient Registration Information Leaflet carefully.

The Management of the practice request all prospective new patients to please pay attention to our Reception/Admin staff when they are giving explanation of the registration process.

Insufficient information on the two registration forms can cause delays as outlined below:

  • Rejection by core NHS Team which will require the practice to obtain the additional information needed for the registration;
  • insufficient information can cause delays in the practice processing the application;
  • inaccurate information may lead to duplication of NHS Numbers or delays in obtaining an NHS number;
  • A patients with NO NHS Number cannot be referred for further care at the hospital when needed;
  • In obtaining records from their previous GP Practice;
  • Later difficulties when patient require a full copy of all medical records to a third party such as registration to be a childminder, to join the army, for Insurance purposes.

The likely difficulties that can occur has been outlined above to give new patients issues that can arise from incomplete forms.

Our less busy times are between 1pm to 3pm.  New Patients are advised that if registration forms are presented outside the above mentioned time, there will be some WAITING TIME.

Please note that the request for proof of ID and proof of address is not a barrier to registration.
